Donna Ismailoglu, MSN, A-GNP
Donna Ismailoglu, came to Senior Care Partners in October of 2017 as an Adult-Gerontology Nurse Practitioner (A-GNP) in the Battle Creek medical clinic. With a background as a hospitalist, it was a natural fit for her to transition into her current role as Senior Care Partners’ Hospital System Liaison. In this role, Donna manages relationships with physicians and care managers within the hospital system and is responsible for collaborating with hospital care managers and hospitalists to monitor the daily care of Senior Care Partners participants who are hospitalized.
Beginning her career in healthcare as a Registered Nurse, Donna has a vast background in various specialties such as med-surgical, cardiac, pulmonary, emergency department, critical care, acute long-term care and neurology. As an A-GNP, she has focused on internal medicine and gerontology. Donna earned her Bachelor of Science in Nursing (BSN) and Master of Science in Nursing (MSN), specializing in Adult-Gerontology, from Spring Arbor University. Additionally, she is a Board Certified Adult-Gerontology Nurse Practitioner (A-GNP) through the American Academy of Nurse Practitioners and is a member of the American Association of Nurse Practitioners.

Nancy Wallace, RN, BS, MA
Nancy Wallace, RN, BS, MA, joined the Portage Senior Care Partners P.A.C.E. Enrollment Team as an Enrollment Coordinator in 2019. Nancy has an extensive nursing background, beginning her vocation as a clinical nurse at Children’s Hospital of the King’s Daughters in Norfolk, Virginia, immediately followed by her role as a clinical nurse at Bronson Methodist Hospital. Over her career she has also been involved in pharmaceutical nursing as well as research, serving as a Research Coordinator for the National Institute on Drug Abuse Clinical Trials Network, Research Nurse for Michigan State University Epidemiology, and Research Coordinator for Jasper Clinic, Inc.
Nancy also has extensive experience in medical writing, researching and writing educational articles targeting pharmaceutical marketing, physician education, nursing education and consumer information. Additionally, she independently researched and wrote a 300-page nursing continuing education course. Immediately prior to her current position, Nancy served as a Hospice Admissions RN for Great Lakes Caring Hospice and as the Integrated Healthcare Manager at Southwest Michigan Behavioral Health.
Nancy earned her Associate’s degree in Science and Registered Nurse certification from Norfolk State University, Bachelor’s degree in Health Science and Master’s degree in Interpersonal Communication from Western Michigan University, and Medical Writer’s Certification through the American Medical Writers Association.
Cathy Johnson, RN
Cathy Johnson, RN, began her role as Enrollment Coordinator with Senior Care Partners P.A.C.E. in 2018, serving the Kalamazoo center. Cathy began her extensive nursing career as a staff nurse and Charge Nurse in the Emergency Department at Bronson Methodist Hospital in Kalamazoo, a level-1 trauma center, as well as at Lakeview Community Hospital in Paw Paw.
She then turned her attention toward home healthcare, serving as the RN Care Manager for Memorial Home Care in South Bend, Indiana and Borges VNA Home Care in Kalamazoo. She also functioned as the Clinical Services Manager for Health Partners, Inc. in Kalamazoo and, immediately preceding her current position, was the Manager of Care Management at ConcertoHealth in Kalamazoo.
Throughout her career in nursing Cathy has served as a preceptor and mentor to others in her profession. She is a graduate of Kalamazoo Valley Community College, earning her Associate’s degree in Nursing and is a licensed Registered Nurse through the State of Michigan. She is also a certified wound care nurse through the Wound Care Education Institute (WCEI).
Nancy Lacey, RN
Nancy Lacey, RN, joined the Battle Creek Senior Care Partners P.A.C.E. Enrollment Team as an Enrollment Coordinator in 2019. Nancy began her extensive nursing career at Leila Hospital in Battle Creek where she worked in the Emergency Room, Orthopedics, and circulated throughout the hospital providing medical care. She then went on to serve as a Case Manager at Battle Creek Health Systems and Southwest Regional Rehabilitation Center in Battle Creek where she performed preadmission and admission assessments, worked with physicians to carry out patient treatment plans and coordinated patient discharge to ensure a safe transition home.
Immediately prior to her current role, Nancy was a Nurse Liaison at Mary Free Bed Rehabilitation Hospital in Grand Rapids where she worked very closely with patients, families and referral sources performing preadmission assessments, coordinating admissions to inpatient rehabilitation, and participating in marketing activities to physicians and hospitals.
Nancy brings a wealth of clinical and patient assessment experience to her current role. Educationally, she earned her Associate’s degree in Nursing from Kellogg Community College and is in the process of completing her Bachelor of Science in Nursing at Michigan State University.
